We aim to provide activities that can enhance the life experience and wellbeing of people living with a Long Term Condition (LTC) and their carers.
We aim to enhance self management techniques, encourage skills development, reduce isolation and provide volunteering opportunities, especially for members who have been out of work due to a long term condition.
The remote Isle of Lewis is situated 40 miles off the North West Scottish mainland. Although a stunning place, living in the Western Isles can cause extra challenges for anyone living with a long term condition and their families. We are here for anyone who needs us.

Self Management focuses on what YOU can do to improve your situation despite limiting or difficult circumstances.
Self Management skills and techniques are simple and easy to learn – but sometimes more difficult to apply as it often means changing what you do or believe or how you think about things.
LTCH members are supported via daily group WhatsApp messages, regular Facebook and Instagram posts. Our YouTube channel, provides a useful resource of Self Management videos and practical tools.
